Employee verification form. In a workers compensation case, this form is a bi-annual report of earnings to be completed by the injured employee. The form is required to be returned to the employer's insurance carrier within 30 days of receipt or benefits may be stopped.

Employee. All natural persons who perform services for another in exchange for a wage or salary.

Employer. The master; the one who employs the services of another and who pays wages or salaries in return for the performance of those services. Employers include natural persons, partnerships, corporations, the state, and governmental agencies of the state.

Ergonomics. The science of fitting workplace conditions and job demands to the capabilities of the workers in order to prevent or reduce certain on the job injuries, including carpal tunnel syndrome and neck and back injuries.

Evidence. Proof of a probative matter presented at a legal proceeding for the purpose of inducing belief in the minds of the jury or judge. Evidence comes in a variety of forms, including testimony, writings, tangible objects, and exhibits.

Exhibit. A paper, document, or item produced at a court proceeding for the purpose of proving a fact.
